چکیده

Background: Rejection is the most common cause of late corneal graft failure. The purpose of this study is determining the prevalence, and defining the role of those factors which may affect the risk for rejection. Methods: The study was conducted on all patients who had penetrating keratoplasty without immunosuppressive therapy with at least 1 year follow up. Demographic status, cause of graft, and types of rejections were detemined. Contribution to graft rejection, persistence of transparency, and rate of failure due to rejection were considered for each factor. Statistical analysis was performed by multivariate regression and ANOVA tests. Results: Two hundred ninety five penetrating keratoplasties were performed on 286 patients (61% male, 39% female). Mean age at keratoplasty was 38 ± 20 years. Mean follow-up period was 20 ± 10 months. Graft rejection occurred at 7.3 ± 6 months (20 days to 39 months) in 31.8% of cases. The most common type of rejection was endothelial (20.7%). Contributing factors were extent of corneal vascularization, regrafting, anterior synechiae, irritating sutures, active inflammation at the time of surgery, additional anterior segment surgery, history of trauma, uncontrolled glaucoma, history of prior graft rejection, recurrence of herpes infection on graft, and eccentric graft (P<0.05). Patients’ age, size of donor cornea, and bilateral transplantation were not significant. Conclusions: The extent and severity of corneal vascularization, anterior synechiae, irritating sutures, active inflammation, regrafting, additional surgery, trauma, uncontrolled intra-ocular pressure, history of graft rejection,

Purpose: To determine the etiologic factors on rejection and failure rates and ocular surface changes after corneal transplantation by either DALK or PKP, in cases of keratoconus (KCN). Methods: In this cross-sectional study, considering the existing data, patients with keratoconus referring to Labbafinejad Medical Center from 2006 to 2016, who underwent corneal transplantation, were enrolled a...

Abstract Background: Although corneal graft may be rejected by the immune system of the recipient it remains as the most successful operation as compared to transplantation of other tissues. Since most patient do not reject the grafts, those who do are in the minority. This study was carried out to assess the usefulness of the cure frailty model for determining the significance of risk facto...

Correlation of Corneal Allograft Rejection with Tumor Necrosis Factors-Alpha Gene Polymorphism

Background: Correlations between bone marrow, heart, kidney, liver, skin and lung transplant rejection or survival with human cytokine gene polymorphisms have been described. There are also reports about the role of cytokines and Tumor Necrosis Factors-Alpha (TNF-α) on corneal transplant in animal models. Further studies are needed to clarify the role of cytokines in corneal allograft rejection...

Corneal neovascularization as a risk factor for graft failure and rejection after keratoplasty: an evidence-based meta-analysis.

TOPIC Preoperative corneal neovascularization (CNV) is thought to be associated with an increased rate of corneal graft failure and potentially also graft rejection. CLINICAL RELEVANCE New therapeutic options that offer differential influence on the ingrowths or regression of either corneal blood or lymphatic vessels force us to re-evaluate the known data about the role of CNV in keratoplasty...
